This edition of "Survive and Thrive" focuses on new Commission on Cancer (CoC) accreditation standards requiring survivorship care plans for all cancer survivors who are treated in CoC-accredited institutions such as Seattle Cancer Care Alliance. This patient-centered approach, which has the strong support of national cancer advocacy organizations, enables cancer patients to become partners in their own care. Providing treatment summaries and survivorship care plans has always been a core function of our Survivorship Clinic.
If you or a cancer survivor you know could use a roadmap of evidence-based care tailored to your specific health history and needs, please contact us. We’d love to arm you and your health care providers with critical information for taking optimal care of yourself after your active cancer treatment has been completed.
